At Connect Healthcare, our diabetes educator supports people living with Type 1 diabetes, Type 2 diabetes, Type 3C diabetes and pre-diabetes, with a practical focus on building confidence in everyday diabetes management.
They can also support clients using or considering diabetes technology, including Continuous Glucose Monitoring and insulin pump therapy.

What does a diabetes educator do?
A diabetes educator helps you better understand diabetes and how to manage it day to day. This may include support with blood glucose monitoring, medications, insulin, diabetes technology, lifestyle changes and reducing the risk of long-term complications.
Do I need a referral?
No, you can book a private appointment without a referral. If you have a GP Chronic Condition Management Plan, you may be eligible for a Medicare rebate if Diabetes Education has been included as part of your plan.
Can I claim on private health insurance or Medicare?
