What is diabetes education?

Diabetes education helps you make sense of your condition, your treatment plan, and the day-to-day decisions that influence your health.

At Connect Healthcare, our diabetes educator supports people living with Type 1 diabetes, Type 2 diabetes, Type 3C diabetes and pre-diabetes, with a practical focus on building confidence in everyday diabetes management.

They can also support clients using or considering diabetes technology, including Continuous Glucose Monitoring and insulin pump therapy.

Diabetes often involves more than blood glucose levels alone. At Connect Healthcare, our Diabetes Educator works within a broader allied health team, allowing care to be coordinated when needed across exercise physiology, dietetics, podiatry and other allied health services.

This can be particularly helpful for people managing diabetes alongside other health conditions, or those wanting a more structured and supported approach to their health.

What does a diabetes educator do?

A diabetes educator helps you better understand diabetes and how to manage it day to day. This may include support with blood glucose monitoring, medications, insulin, diabetes technology, lifestyle changes and reducing the risk of long-term complications.

Do I need a referral?

No, you can book a private appointment without a referral. If you have a GP Chronic Condition Management Plan, you may be eligible for a Medicare rebate if Diabetes Education has been included as part of your plan.

Can I claim on private health insurance or Medicare?

You may be able to claim a Medicare rebate if your GP has included diabetes education as part of a GP Chronic Condition Management Plan.

Medicare rebates depend on your individual care plan and referral, so we recommend speaking with your GP to confirm whether you are eligible before booking.

Some private health insurers may also provide rebates for diabetes education under Extras cover, depending on your level of cover. We recommend checking directly with your health fund before your appointment to confirm whether diabetes education is included in your policy.
