The 6-Week Diabetes Reset Program at Connect Healthcare brings together the expertise of Diabetes Education, medicinal advice and Exercise Physiology, supported by Continuous Glucose Monitoring (CGM) technology, to help make diabetes management more understandable, personalised and achievable.

Over six weeks, you’ll receive individualised support to turn information into practical action, with strategies tailored to your health, lifestyle, fitness level and personal goals.

What’s included in the Diabetes Reset Program?
The Diabetes Reset combines four Diabetes Education sessions and three Exercise Physiology sessions, providing 7 individual sessions across the six-week program.
4 Sessions with a Diabetes Educator Pharmacist
Your Diabetes Educator & Pharmacist will help you better understand your Type 2 Diabetes and the factors that can influence your glucose levels. You’ll receive personalised education and practical support to help you understand your condition, your medications and the role your everyday lifestyle choices can play in managing your diabetes.
3 Sessions with an Exercise Physiologist
Your Exercise Physiologist will help you develop an achievable exercise plan tailored to your current fitness level, health and goals. The focus is on helping you become more physically active in a way that is appropriate for you, while building the confidence and strategies needed to make exercise a sustainable part of your lifestyle.
Real-time insights into your glucose levels
As part of the Diabetes Reset Program, you’ll receive a complimentary Freestyle Libre 2 Plus Continuous Glucose Monitor (CGM).
A CGM provides real-time information about your glucose levels, helping you see how they change throughout the day and providing valuable insight into the impact of your lifestyle.
Together with support from your Diabetes Educator, this information can help you better understand your individual glucose patterns and make more informed choices about your diabetes management.

$630 for the complete 6-week program

- Personalised education and exercise support throughout the program
Approximately $317 back from Medicare*
Eligible participants may be able to claim Medicare rebates for appointments under a GP Chronic Condition Management Plan (GPCCMP).
For participants eligible to claim the applicable Medicare rebates, this can reduce the overall cost of the program to approximately $300 out of pocket*
*Medicare rebates are subject to individual eligibility and the number of eligible allied health services available under your GP Chronic Condition Management Plan.

Diabetes Reset Program FAQs
Do I need a GP referral?
You don’t need a GP referral to participate in the Diabetes Reset Program. However, an appropriate GP Chronic Condition Management Plan is required if you wish to claim eligible Medicare rebates for your appointments.
What if I don't have enough Medicare visits available?
You can still participate in the program. Medicare rebates depend on your individual eligibility and the number of allied health services you have available under your GP Chronic Condition Management Plan.
Private health insurance rebates may also be available depending on your fund and level of cover.
What is a Continuous Glucose Monitor?
A Continuous Glucose Monitor, or CGM, provides information about your glucose levels throughout the day. This allows you to see your glucose patterns and gain greater insight into how your lifestyle may be affecting your levels.
The program includes one complimentary Freestyle Libre 2 Plus CGM, with an additional CGM available for $15.
Do I need to be exercising already?
No. Your Exercise Physiologist will work with your current fitness level, health and goals to develop an achievable exercise plan appropriate for you.

All Diabetes Reset appointments are held at Connect Healthcare in Norwood.
How much does the Diabetes Reset Program cost?
$630 for the complete 6-week program, with approximately $317 back from Medicare, subject to individual elegibility and the number of eligible allied health services available under your GP Chronic Condition Management Plan.
