If you are attending Connect Healthcare for the first time, you will be asked to complete a clinic intake form and a relevant pre-consult form. For existing patients seeing a practitioner in a new discipline for the first time, you will be asked to complete the relevant pre-consult form. The information you provide will help your practitioner gain a thorough understanding of your condition and previous history.
We ask new clients to arrive at their first appointment 10-15 minutes early to complete pre-consult paperwork, but if preferred we have provided the relevant forms below for you to complete in your own time at home.
